When creating a custom service request form in the Service Catalog, all the fields are in 2 columns and not able to space them. it would be nice to create the fields into sections.
for example:

Complainant Name, Address, Phone, E-mails
Issue/Complaint Information:
Date Received
Time Received
Specific Location
Date Occurred
Time Occurred
Individuals Involved
Issue/Complaint Details
Disposition of Complaint
