Add Questions and Rating Scale in Customer Satisfaction Survey Feedback Page

When we click the Satisfied and Dissatisfied button, we are redirected to this page. This is where the requestor adds a comment. Is it possible to customize the content for us to add some more questions and types of responses like rating a scale t measure responsiveness, manner of responding, expertise, etc.?
