I think everyone would agree that its essential to get as much information regarding a problem from an end user as possible. That means tons of fields.
I think we can also agree that there is nothing worse than a messy SUPER LONG form that keeps going and going!
What SWSD needs is an ability to make multi-page forms where fields could be formatted and located in any way that we need them to be displayed - something like JotForm...
IMO, this is essential to an affective UI for a HelpDesk system.
