Add Custom Fields to Service Catalogs

Currently we can add Custom Fields under Inputs, but we cannot add a Custom Field that applies to all service catalogs.

We do not want to add another field that needs to be filled out by the submitter, we want this field preset like the categories, site, dept., etc.

@TimdelaRosa I just looked and you are right that we don't present the ability to say Service Request or Incident in the conditions of a dynamic form. We'll have to look into adding it. In the meantime you can add the fields you want filled out by service agent users to any form that will be displayed for the category that the service request will fall into.
