Custom forms/fields should be displayed directly under the fields that triggered them, not all the way at the bottom of the page

Currently, if a field triggers a custom form to show via Dynamic Form Rules, that custom form is displayed all the way at the bottom of the form, which doesn't make any sense!

The form should be displayed directly underneath the field that triggered it.

For instance: If someone chooses Category 'Applications' and Subcategory 'Email"- show a drop-down menu with additional questions as to which email problem they are having..

This is huge for us too. These fields should be right next to each other and you should be able to design your incident ticket the way you want to. Needs to be more customizable.
