Experiencing the same issue and can confirm that we have a safe link service with Microsoft, so that is likely the culprit? But is there a way to resolve this?
The In-Notification Direct Actions option should help (it should be set to the Off position):
Will turning this off remove the approval notification buttons that say Accept or Deny? wouldn't want to get rid of those if that is the case.
Yes, those buttons will move from the email to the survey page. The survey notification email will include a single button that will take requesters to the survey page.

I did yes, found it more beneficial to have surveys working than not!
