I have created a very simple custom form on our SWSD, but I don't see the module form field noted in the documentation (step 3). Hopefully, I am missing something simple!
Hi @spb87, that documentation is no longer accurate. They updated custom forms on 4/3, and haven't updated their main documentation yet. You may be able to use these release notes in the meantime:

The short version is that you'll create the Custom Form, and then apply it to different modules using the Dynamic Forms Rules in Setup > Service Desk > Dynamic Form Rules
