After logging in to answer the survey the following page comes up. Does anyone know what I need to do to fix it?

Likely a known issue, log with the Support
It's an issue I reported months ago. Doesn't seem to be a priority for anyone at SolarWinds to fix though. Definitely report it...the more people whom complain the more likely someone will take notice.

HiWe're new on the WHD and try to test the survey function.Web Help Desk Version 12.7.12 - Build #12.7.12.1710We create a new survey and send to one of our test user. He get the e-mail and also the link.As soon as he click the link, he get the same error as already reported.Text in the browser: <html><head><title>Error</title></head><body>Your request produced an error.</body></html>Possible Solution:As I try the link on the same computer, which I logged as tech, I logged out and click again to the link.Now the system, say me, I need to login. I login with the test user and see, the survey come up.Best regardsSalman
@SalmanS logging in as a user is one solution, but that never used to be required. Anyone who has conducted survey's in the past know that being able to submit surveys anonymously is pretty important.
More importantly, not every Web Help Desk customer requires their customers to login to get help. We for example, allow users to get help simply by sending an email to one of several email addresses (i.e. one of each product).
So if you operate Web Help Desk in a way where you don't require your customers to login or even have an account in Web Help Desk, the "new" method for collecting survey data fails.
That means in situations where customers whom have no idea we use Web Help Desk and weren't issued an account, need to submit a survey...you now have to issue them an account.
The design was changed...which caused the errors we're seeing and made customers like myself, have to consider changing how we do business. Literally every other Help Desk system out there allows you to collect survey data without needing an account.
Web Help Desk should allow this as well...and not require the added complexity have needing to login just to submit a survey. That requirement to login? Has cut our survey responses down from almost 100% submission...to just 5%. Having to login in order to submit a survey is TOO large of a barrier to users...so they just don't give feedback now.
It was a bad choice to change Web Help Desk that way.
Are you aware a Hot Fix 1 is available for the 12.7.12 release, not saying this will resolve but the first thing to try?

Do you run over https with a certificate applied?
WHD12.7.13 is out now, might be worth upgrading to see if it is resolved?
We do! Fully functional in that way.

The issue hasn't ever been changed/fixed - we are at 12.8.1 - Build #12.8.1.758 and we get even a different behavior now. This error just shows up no matter who you are when you attempt to submit a survey:
