I want to add a notes field so that its possible to leave comments on an alert. I'm fairly sure this isn't possible but I just thought I'd ask if anyone knows a way to do it.

It is not possible to add input fields to any widgets in modern dashboards.
As someone with loads of custom alerts widgets, the normal alerts page is your friend and anyone who says otherwise is wrong
You could pull up the already-input notes though
