Currently we make notes on a down alert log entry (usually these notes are short and describe the outage, e.g. power loss, ticket #xxxxx opened with ATT, etc...):

Most of our outages are power related. When we run our monthly availability reports I would like to include the notes in the report so we can identify the cause of the outage (power, ATT, etc...). Has anyone implemented this sort of feature in their outage reports?
