I had this issue and found that for me, it was related to copying an alert. If I copied an alert, the alert that I created by virtue of the copy was fine and included all the alert actions. However the alert that I copied from, had it's alert actions missing. I've replicated this time and time again... much to my dismay....

What about dependencies ? I would be great to have the ability to create device groups, then assign dependencies to the group as a whole - i.e. all switches behind a router at a site are dependant on the router.
