How to create a custom template to monitor a SQL Server?

Hi,

We have SAM 6.2.4.

Although there is AppInsight for SQL template for SQL Server. I would like to create a new template with a simple check to see if instance is up or not.

Questions:

1. What is the best way to accomplish this?

2. I went to create a new template option but couldn't figure out how to create a SQLServer related check?
