Storage Resource Manager - LUN Monitoring

The following condition we want to be deployed in SRM module for LUN monitoring -

Could you please help to deploy this condition

"Response time >20ms for Continue 5min OR response time >20 ms within 30 min 15 number of time (mean if we identified 15 spike within 30 which are > 20ms)"
