I would assume that you added this via SNMP, is that correct? If it's a linux server, try using agent Polling.

Below are the OIDs for CPU and Memory. If your Linux server does not support those OIDs, then you need to install the Agent into the Linux server.
You need to remember that SNMP is mainly used for Networking Device, for Windows, it would be via Agent Polling or WMI.
