Can you try from another browser? Wondering if it's just rendering odd in the one you are currently using...
Yep I've seen it before in IE10, same page was fine in Chrome

Unfortunately, in our Citrix environment we are only allowed IE.
*gives me an idea though...
I have the same issue and opened a ticket for it.
Using IE 11 with what appear to be the same settings, I get the issue when connecting to DPA under Citrix, but not when connecting to DPA outside of Citrix.
It looks like a Citrix/IE specific issue - did you get a resolution?
