It's the human factor that scuppers us most often, which actually stems from the lack of automation and the lack of a decent CMDB.
Lack of consistent/uniform standard. Our only consistency is that we are inconsistent.
Difficulty sharing knowledge.. we want to teach, we provide KB articles.. we can't make someone read them.
agreed it is magical information capture if someone leaves to extract there knowledge with there consent. Some mind probe might be handy, a solar mind probe....maybe
Lack of funding is a consistent issue.

Difficulty with old-school tools: missing Terraform provider, incomplete API endpoints, outdated framework, slow development, inconsistent/illogical interface, poor support, and ignored feature requests...
Certain individuals hoarding information to "protect" their jobs. No teamwork and people in roles they are entirely unsuited and unqualified to be in. Oh what fun.
