How can I create a custom report to bring back my worst performing nodes from the last 3 (X) months?

The following report might be what you are looking for:
A user has created resources to show the same information in the web console:
Alternatively there are some default Node Availability Reports in Report Writer:
Start > All Programs > Solarwinds Orion > Alerting, Reporting and Mapping > Report Writer
Expand the "Daily Node Availability" group and select the "Availability - Last Month" report.
The first custom reports is more detailed, then it depends what you need.
