Monitoring Multiple Agents With the Same IP Address

I have two server one at HQ and the other one at DR on (active Passive mode), Both servers are sharing the same IP address and Hostname, When I do failover the agent goes offline. Anyone with recommendations on how to pool them without the agent going offline
