Is it possible to fire an additional alert after the 1st one, under the condition that if it's not acknowledged within X amount of time, someone else is emailed/paged?

Might be worth checking this app out to add on
you could just keep repeating the first alert - Under the action definition, Alert Escalation tab, Execute this action repeatedly while alert is triggered and check box Do not execute this action is the alert has been acknowledged.
Or, create another action that is delayed (same tab) with the repeat till acknowledged.
