Cisco Switches Not Showing up correctly.

I have problems adding our cisco switches to the NPM. Some of them will be added correctly, however others will get added via IP address and then will somehow no longer keep their name as the IP addy.
The above shows how it becomes renamed as a blank space in the database, I am *NOT* renaming it, it just automatically becomes a blank. This results in the following on the website.

Sometimes I can rename the item via the console which will force it to work, however sometimes it will not rename at all and just stays as a blank space. I have uninstalled Orion and the databases and recreated them from scratch and this still occurs, I have also applied the 3 patches from the sticky.
I spoke to someone from tech-support however they did not give me a case #. Please advise.

Event log shows that it was added by IP address. If I repoll the device it will show the name as the IP and The correct IP, however the device still shows incorrectly in NPM and Website.

Bump, still need any input on this. This is occuring with the latest build of NPM.

The name usually comes from the prompt on the switch.
Make sure this is set correctly.
Alternatively, you can rename the node inside Orion to show the correct details.

Telnetting to every switch in our enviroment the Prompts are IP address>
ex 10.50.48.5>
I am also unable to rename the device.

Issue is still occuring, The very first switch I have added to a brand new blank database and it does the same as above.
You can see the database is not being properly populated,
This occurs in both MSDE and MSSQL Express databases.

Could this be because periods are illegal characters in the hostname? This is perfectly legal in a FQDN but hostnames cannot contain periods. Maybe the developers didn't envision anyone using the IP address as the hostname of their switches. Just a thought.
