Interfaces appearing twice and unknown interfaces starts appearing for a node in NPM

Can any one kindly tell me why all the nodes configured in NPM turns grey frequently(say after every 5 minutes). Another issue is that some interfaces are present in some nodes which does not belongs to them. For example node X does not has Serial0/1 interface but in NPM it shows this interface and it is in unknown state(Grey color). Since this interface does not belong to the concerned node there is no way to delete this. Only option left is to delete the node and then reconfigure it. Some interfaces which belongs to the node and configured appears twice in NPM. But in the list resourses it appers only once. Why it is appearing twice in the NPM

Kindly tell me why this is happening and what logs to check to identify the problem.
