network baseline report

does anyone have a sample of a network baseline report they could share? I am looking at making a report that would have some of the following.

Circuit Name
Circuit Config Bandwidth
ping times
Peak Bandwidth 30 Days
%Average Receive 30 days
Average Send 30 days
Peak Cpu load
