Creating DropDown List

I'm currently trying to create a Custom View for adding change control notes. The page requires a dropdown list of nodes, plus a set of simple text entry fields for user-supplied info.

Easy enough, but I'm not proficient enough to program the dropdown list properly. I imagine I'd populate it using NodeID and Caption, and use 'Caption' to generate the list.

Question: is there any template I can use to create such a list? Basically, I want the user to choose an available node, fill in the rest of his info, then hit the 'Submit' button at the end.

Any ideas would be appreciated. I imagine this is pretty simple coding, but it's not something with which I'm familiar.
