Can Orion Network Performance Monitor alerts be configured to ping router, etc before paging?
You can have the alert trigger a page when a certain condition has held true for a certain amount of time.
So how would I go about creating an node down alert and have it ping the router to see if it's a network issue. Retry the node and send alert page if condition still exist after 15 minutes

Orion will continue to ping the device even after it does not respond.
You do not need to set this up with an alert-
see this doc. for a walk through setup:
Trigger Alert when all of the follwoing apply
Node Status is not equal to up
Alert Escalation Tab- delay the execution of this action 15 minutes.

It sounds like you want to suppress the alert if the router is down, so this should also be included in the alert configuration as well; either as an actual suppression, or as part of the trigger condition itself.
