Help needed with Orion NPM report on device adds/removals

Hello,

My manager has requested for me to provide a report that is not available (out-of-the-box) from the Orion Report Writer. So, I am writing to you, in hopes that someone else in this forum has successfully gotten a report like the one below:

The report we are seeking for would list all device adds and removals during the last 30 days and it would show the login ID for the person who performed these. Obviously, we are trying to figure out who has been adding and removing devices from Orion website. My hope is that there is a SQL statement that can easily be applied to get all this info from the Orion NPM database.
