Nightly Maintenance Never Completes

My nightly maintenance gets stuck on:

It never gets past this phase, and the process never ends. Meanwhile, the avg disk queue length on my DB drive goes from <1 up to 30 and stays there until I kill the database-maint.exe process.

Has anyone seen this?
