I am trying to monitor cisco 6500 series modules.Is there any one is having templates for this or procedure how to monitor and alerts.

Is there something specific that you wanted to monitor and alert on? (IE: Ports / Backplane / Errors / Counters / etc.)
Here are links to three universal device pollers that I know of that might help you get started. These will give you an inventory of the modules in each slot, backplane utilization and fabric utilization of your 6500.
