Packet loss algorithm?

I've got a question for the crowd.
Does anyone know how SW determines packet loss for a given end site? From what I've been able to determine, it looks to be more of a decaying average over time, but I need to figure out just how that works in order to make an actionable alert on it.
Anyone have any ideas?

I believe it is simply how many of the last 10 node polling cycles have failed. Each missed cycle increments the counter by 10%

I wonder if that includes the fast polling packets as well?
Also;
Is there just one ICMP packet sent for each poll, or does it send 3 in quick succession
for each poll?
If no one has tried this yet, I can always sniff my network for the correct answer...
