Service-Now: How to do an automation of the deployment of Application Monitor Template?

We are using Service-Now and we received a requirement for the automation of the deployment of Application Monitor Template.

I assume you mean having a service-now form that a user fills out and then communicates to Orion to create an application monitor/setup alerting?

I am interested in this too.
