how to create a new Template for; SQL Servers and Web Servers in SAM.

Go to Settings>All Settings>PRODUCT SPECIFIC SETTINGS section. Then select SAM settings and go to APPLICATION MONITOR TEMPLATES section. Then click on Create New Template and have fun .
However I'd suggest to select one of existing SQL templates and modify it as you wish.
