Monitoring requst form?

Anyone have any example templates used for server / app monitoring requests? I am setting up a new workflow and hate building it from scratch. This form would be given to the parties who desire monitoring, they will then fill out the info and send to me to have added to Orion. I am being lazy and trying to avoid starting over.

I'm actually working on something like that now for our ticketing system. I've attached what I'm using to send to our team to have added to our ticketing system.

This is pretty cool. Do you have them fill out one form per server? Or can they list a number of servers with similar needs ( say, 30 Oracle servers, monitor disk space and network up/down for each ) on a single form?
