I am in a situation where my SQL Server is listening on a port other than 1433, how do I configure a poller to look to an alt port? I have tried hostname:10999 and it does not work.

When you assign the SQL Insight app monitor template to your node the below options are presented during the Assign application monitor wizard..
Set up AppInsight for SQL on which node
SQL Server Instance Name SQL Server Port Type Use default port Use static port SQL Server Instance Port SQL Server Credentials
