Unable to add AppInsight on node discovery

I have SQL nodes monitored with WMI. The service account used for polling granted SA in SQL. I am unable to add AppInsight as a resource, yet I am able to apply the AppInsight template manually while using the node polling credentials.

When browsing resources on node discovery, AppInsight and instance names appear available. After selecting the instance, AppInsight is not applied.

I also notice SQL error log filling up with "Login failed for user 'xxx\xxxxx'. Reason: Attempting to use an NT account name with SQL Server Authentication"

Are there adjustments I can make to allow application of AppInsight at node discovery, and/or adjustments I can make to stop the error?

First, ensure that you're not assigning AppInsight like you would a template. Start by deleting/removing AppInsight for any node it is not functioning properly on. Next, upgrade to SAM 6.0.1. It can be downloaded from your customer portal. After applying SAM 6.0.1 and go to the "Node Details" view for the host where MSSQL is installed and click "List Resources". Select the SQL Server Instances from the list you'd like monitored and click submit.

From here, find your way to the "All Applications" tree and click on your newly monitored AppInsight for SQL instance. If a dialog window opens and asks you for credentials, provide your SQL Server SA or equivalent credentials. If it does not, wait up to 10 minutes to see if polling is working properly. If after 10 minutes AppInsight for SQL is still not collecting data click "Edit Application" from the AppInsight for SQL Application Details view. From within the application editor validate the credentials being used to poll AppInsight for SQL. It may be that there is an issue with the credential type, (SQL vs Windows). From here you should be able to test your credentials and determine what the issue is.

Please note: AppInsight for SQL connects the to SQL Server using the SQL protocol, not via WMI or RPC. If you SQL Server is not configured to accept Windows authentication, you will need to provide your SQL  specific "sa"credentials before AppInsight for SQL will be able to poll/collect data properly.
