Disk space Alert not firing for one Windows server

So, I have a Disk Space alert which I copied off of the Out-of-box alert from Orion and am using it for number of months. For the most part it works fine and then once in a while, it misses out on 1 server at random. I am not able to drill further on why this happens. Are there any DB tables or logs I can look at which specify the Disk size or an attempt to fire the alert, etc? At this point, I am simply saying to myself "oh well, it didn't work". I need some help to take this to the next level. Remember, the Windows servers it misses are at random. Below is alert screen shot. These are VMware guests.

Can you post a screen shot of the disk % utilization for one of the misses, for the time period where it was missed?

Also, do you have a delay on this alert (it would be at the bottom of the alert trigger tab). Generally my experience is NOT to trigger alerts after 0 seconds because there are always blips.

Also, specific to disk monitoring, SolarWinds only collects disk metrics every 10-15 minutes, so it's completely possible that if you had a momentary spike you would miss it.
