Where can I find SQL Server User Experience Monitor?

I have seen various posts where individuals discuss running a query against a Microsoft SQL Database and then create an alert that is triggered based on the result of the query.  My issue is that I am unable to locate the "SQL Server User Experience Monitor" template that is referenced in these posts; it does not appear to be a default template included in APM 3.5 (I do see MySQL under User Experience, but no Microsoft SQL User Experience Monitors).  Can someone help me locate the template and/or post a link to where I can download it?

If you need to issue custom SQL against SQL server and use this value for alerting and reporting, you are looking for "SQL Server User Experience Monitor". Go to Admin -> APM Settings -> Create a new template -> Add component monitor -> User Experience Component Monitors -> SQL Server User Experience Monitor.
