Issues trying to monitor a MYSQL database

We have several instances of MYSQL running on 2 Windows servers. When adding one of the Windows based MYSQL servers to APM, I chose to assign the MYQSL application monitor. I created new credentials using the MYSQL Database admin account. When I test the credentials, I get the following error message for all tests:
