Failed to retrieve the list of installed modules

When running Active Diagnostics on our Orion server, we get the error "Failed to retrieve the list of installed modules". We get this error for three of our ten APEs, and one of our three extra web servers. Ideas, anyone? Thanks, Brent

Thanks, Neo, I'd seen that. However, our web server table does not contain any old web server names.
