Requirement Gathering Template

Hi All,

One of our customers has purchased NPM, NCM, SAM, Netflow products. They want us to do the installation as we support their existing infrastructure. Could someone please provide me the standard require gathering template which includes system requirement questions, Databases, network, elements, sizing the environment etc.
