Virtual servers displaying incorrect name and IP

Today I noticed I had some duplicate servers on a couple of our virtual hosts. Initially it just looked like it was the same server, but if I hover over the server name, it displays a completely different server and IP. If I click on the server it redirects to the device displayed when I hovered over it, not the server listed. This seems like a symptom of how the VMs are managed on the host, but I can't explain how or why it's doing this. I'm not sure how to prevent this from happening, obviously it's misleading and will cause confusion to whomever tries to view the servers.

I do not have the Virtualization Monitor configured, this information is being monitored through the default VM monitor in SAM.

Is these hosts running VMware or Hyper-V? If VMware, is this information being polled through the vCenter or directly from each ESX host?
Are the VMs themselves managed nodes in Orion, and if so how are those nodes managed? SNMP, WMI and Agents or just ICMP?
Any any hosts of virtual machines duplicated in your monitored environment, such as a production vs staging, etc.?

1. They are running Hyper-V

2. The virtual servers are managed via WMI

3. Each server has a unique name and IP, there shouldn't be any with the same information. We have multiple environments, development, developmental test, test, staging and production. Each has it's own naming convention and IP range to distinguish between them.

I'm not sure if that is the answer you were looking for with regards to #3.

We have clustered hosts to enable failover capability so the servers will occasionally move from one host to another.

Do those servers which are incorrect or are duplicated share the same MAC Address in common?

They're different mac addresses.
