Unknown interfaces after reboot on Cisco 3640 router

I've got a 3640 that got restarted and now two of its (still functional) interfaces show as Unknown. The rest are OK. I've checked the config and 'snmp-server ifindex persist' was already in place. What might be causing this? The IOS seems to be 12.2(15)T12, but show version seems to say its 12.2(100).
