I have the SQL query working properly, but I am not sure what is the proper trigger and reset actions I should configure for this alert. Could someone please tell how they did it?

I'm not aware of a way to get NPM to recognize a port is in err-disable state. However historically when wanting to be notified of this I use syslog alerts to send me syslogs that contain *err-disable* in the message.
