It looks like this monitor is not available. This might be a dumb question but can I use the HTTP form and just change the port and use an HTTPS URL?

Andy - yes, the form login monitor supports HTTPS (it does not need a separate component like the http/https website monitors). If it's failing, most likely it's because it's a javascript form and not a true HTML form.
I see. Is there and path forward with a java form?
You would need WPM for anything with java.
Forms that utilize JavaScript need a full fledged browser with a JavaScript engine. For this purpose (and much more) Web Performance Monitor was born. I recommend you give that a shot.
