MSSQLSERVER Login failed applying SQL Server 2005-2008 Performance template

I have applied this template and can sucessfully test the credentials and the template returns results from the monitored components.  However, my application logs are flooded with Failure Audit attepmts from the SolarWinds server with the user applied in the template.

Any ideas?

Also SQL Server User Experience Monitor uses provided credentials for both - Windows Authentication and SQL Authentication. Order, in which these methods are tried is driven by option in Monitor settings (on Application Edit page). Therefore maybe just changing this option could resolve the issue.
