Alerts - cpu 80% memory 75%

anyone know how to create an alert for one of our vm servers for thresholds of cpu 80% and memory of 75%. it is our busiest time of year and already the director of the data center was called in today due to a memory issue. he asked for this alert asap. any help is appreciated. thanks

Just to clarify do you need the condition to be both of those thresholds at the same time of just either one at a time?
