I've been having issues with custom thresholds staying in place for a server in the SQL App Monitor. I'll adjust the thresholds for a Custom Performance Counter Monitor usually based off the recommended Latest Baseline Thresholds details section but then at some point in the future, the thresholds revert back to the default that was there. Sometimes the changes I made stick for 5 minutes, a day, or a week. However, they all revert back to their defaults at some point. I have no idea why it's doing this.
An example would be I adjust the Page Writes/sec perfomance monitor from a Warning alert of greater than 60 and a Critical alert of greater than 90 to the recommended warning alert of greater than 115 and a Critical alert of greater than 160. The alerts are set for single poll and I do make sure to click submit and wait for the screen to gray out and come back to normal before doing something else on the page or exiting out.
