Duplicate Top_Level domains

I couldn't find anything from past discussions on this, so I thought I'd post thew question.

When I add new nodes to the NPM, I am getting duplicate top_level entries when i look at my side panel. When I added the nodes, I added the top_level and third_level with the exact spelling etc... but the NPM created a duplicate top_level. Has anyone run into this issue before?
