How to setup an Alert to monitor Only One of the AppInsight for SQL components?

Hi Guys

I have been asked to create an alert to monitor our Critical SQL Servers/Clusters - Node Down, Low Disk Space, CPU Util, and Average Lock Wait Time.

All of the above bar the last one is fine, I have the Node Down for a given period to trigger, Low Disk Space to trigger as per thresholds, CPU Utilisation to alert if running at 80% Util for 120 seconds or more but I am stuck with the Lock Wait Time.

I can see the AppInsight for SQL template which has been assigned to a number of our nodes by someone else but I have a list of 6 Critical SQL Cluster servers and only want to monitor the Average Lock Wait Time component out of the 52 other SQL components.

How do I setup an 'Average Lock Wait Time' alert to trigger when the Lock Wait Time is for 120 seconds or more?
