You can achieve that by using Node Details Report under Web Based Reports

Do you know what the function would be to create the report from within the Orion Report Writer?

I am really not sure where is it picking the description from (Application 'BPL Production XXXXX' ) - that's the reason I suggested a Web Based Report to achieve this, if you create a Node Report from Report Writer you would only see if Node is UP/Down under the description

are you just asking how to create a report that shows application health per node?

Would I would like is to have a report that shows the down applications and shows what part of the application is down. That way I can send the report to the sys admins and they can fix the issues

Take a look at the 'current status of each component' report in the legacy report writer utility. You should be able to save it as a new report and add in a filter on component status not equal to Up. You can also mimic the same in the web reporter.
