Switch Interface Status Report

I want to design a report that will allow the selection of the switch by the person running the report.

The information I want in the report is switch name, ip address, interface id and last interface status change

and I only want data for the report where the last status change occured 180 days ago and the interface status is currently down.

I've tried multiple different designs but cannot get the exact report I'm looking for.

If anyone can point me in a direction to get the report I'm looking for would be great.

I would personally direct users to the event manager and teach them to query the logs. If your vision is a requirement your best bet is SQL SSRS. Not sure if it's possible to have users define the node and have information reported back, unless you write custom reports for every switch you have and grant users to that webpage.
