Heavy Packet Losses

Hello,
We have a server hosted with one of the hosting providers. We are facing very heavy packet losses everyday, and also heavy memory usages everyday.
We have tried reducing the disk space usage to 22%, and compacted the database etc, and whatever we could do to reduce the load on the server.
After all these we still don't know why the packet loss is caused ? Any ideas. First is it that its a hardware issue or a software issue ?
Any suggestions.
